Payment Authorization Form

To apply a payment to your travel itinerary, please fill out this form to authorize RISE Global Travel Company to manually submit your payment to your itinerary balance. Please complete all the required fields. This authorization will remain in effect unless cancelled. Please note that payments are only authorized during our hours of operation, unless discussed otherwise.

I, the above named authorized card user, grant RISE Global Travel Company, authorization to charge the card on file for the purposes detailed above. I understand that this form's a legally binding contract and that by signing my signature to this from, I will be held accountable for all agreed upon charges. This payment method is authorized only for the charges listed above. I understand that this charge will be manually applied to my reservation by RISE Global Travel Company, and a receipt will be sent upon payment.

Please note, that you will not see a charge on your account from RISE Global Travel Company, but from your vacation supplier directly. Payments may take 3-5 business days to fully process and to be reflected on your account.
